Notice of Privacy Practices
The notice explains how your health information is used and shared at Atlantic Psychiatric Centers, and the rights you have over it. Ask at any office and we’ll give you the current copy.
How to get the current notice
Three ways, all of them quick.
Call us
Ask for the current Notice of Privacy Practices and we’ll mail or email you a copy the same week.
Ask at any office
A printed copy is kept at the front desk in Palm Bay, Suntree and Rockledge.
At your first appointment
You are given the notice to read before care begins. What else to bring is on New Patients.
What the notice covers
Federal law gives you these rights over your health information. The notice sets out how we honor each of them.
Get a copy of your record
See or get an electronic or paper copy of your medical record, usually within 30 days of asking.
Ask us to correct it
Ask us to correct information you believe is wrong or incomplete.
Ask for confidential contact
Ask us to reach you a specific way, or to send mail to a different address.
Ask us to limit what we share
Ask us not to use or share certain information. We will say yes unless it affects your care.
See who we shared it with
Ask for a list of the times your health information was disclosed.
Get a copy of the notice
A paper copy, promptly, whenever you ask for one.
Choose someone to act for you
A guardian or medical power of attorney can exercise your rights on your behalf.
File a complaint
Tell us, or file with the U.S. Department of Health and Human Services. We will not retaliate.
Some uses and disclosures do not need your permission — treating you, getting paid, running the practice, and situations the law requires, such as public-health reporting. The notice sets out each of them.
Other documents
The rest of our legal and patient-facing documents.
